MetLife Vision Plan
Physical health is important to overall Well-Being and eye health is an important component of physical wellness. Kean University full-time employees have the option of enrolling in MetLife’s Vision Insurance with the Superior network of providers.
The network of providers includes professionally certified optometrists and ophthalmologists, at private and retail locations. Familiar retail locations include American’s Best, Costco, LensCrafters, Target, Pearle Vision and Walmart among others. There is also access to online in-network eyewear stores including ContactsDirect, and 1-800 Contacts.
In addition to coverage for comprehensive vision exams and ways to purchase glasses or contact lenses, the plan provides for a discount on LASIK procedures.
The Superior Vision Plan Summary outlines the in-network benefit levels and frequency for:
- Eye exams
- Frames
- Lenses
- Lens enhancements
- Contact lenses
While it is more cost effective when using in-network providers, a reimbursement may be made when services are obtained by out-of-network providers. The summary also provides the reimbursement schedule when out-of-network services are obtained and the list of plan exclusions and limitations.
No ID cards or claim forms are required when you use in-network providers.
Eligibility
Elections are made annually in the fall during a determined annual enrollment period, to be effective January 1 of the upcoming year.
Similar to health insurance, full-time employees, spouses, civil-union partners, and same-sex domestic partners and dependent children are eligible for vision insurance coverage.
Newly hired employees and their eligible dependents may be covered after 60 days of continuous employment. Full-time faculty members hired as of September 1 may be covered as of the beginning of the academic year. Full-time faculty members that begin employment on other dates are eligible for vision insurance following a 60-day waiting period.
Cost of Coverage
Vision insurance is a voluntary benefit. Employees who choose this coverage pay the full cost depending on who they choose to cover. Employees pay through pre-tax payroll deductions.
Monthly premium for plan year 2023:
- Employee only: $8.48
- Employee + Spouse: $16.96
- Employee + Children: $19.69
- Employee + Family: $30.26

*Vision plan employee premiums are annualized, and costs are spread throughout the year. 12-month employees will have deductions over 26 pay periods and 10-month employees will have deductions over 22 pay periods.

MetLife’s vision insurance does not replace the Vision Care Reimbursement Program. Employees with health coverage through HORIZON plans generally have coverage for one annual routine eye examination by an in-network ophthalmologist or optometrist subject to applicable co-payment and or deductible. Make yourself informed about all of the ways available to receive coverage for your eye health.
Vision Plan Members
If you electing vision coverage through MetLife, Superior network, you may see your coverage election, as well as those eligible family members you chose to cover, in Workday.
To find providers participating in the MetLife Superior network or to view a digital ID card, you can download the MetLife US app on your smartphone. You will need to register. Once you register, you will have easy access to finding participating network providers. When you make an appointment and see a participating provider, you do not need to present an ID card. You only need to identify that you have coverage through the MetLife Superior network and the provider’s office will do the rest.
To see details about your coverage in advance of your appointment with a provider, access MetLife online and register at online.metlife.com. Choose My Accounts and obtain information about your Superior Vision by MetLife benefits. Select “I want to view my policy details” to find out what’s covered and the coverage levels when you see an in-network participating provider.
After you receive services, such as an eye exam or the purchases of frames and lenses, you can check back and verify your claims. You will also be able to access and print an EOB. If you have coverage through a healthcare flexible spending account such as Tax Save through Horizon My Way, you can submit to that plan for reimbursement consideration for any of your out-of-pocket expenses or use your Horizon My Way debit card at the time of purchase.

*Please note that MetLife offers three vision plans - Superior, VSP and Davis. If you call the main customer service number of 1-800-GET-MET8 you must select the correct plan to be routed to the correct customer service team. If you select one of the other plans in error, you may be informed that you do not have Vision coverage as they will not find the enrollment. To avoid this, please always indicate you are a member of the MetLife Superior Vision Plan when you call MetLife.
