Exemptions for Immunization Requirements

Request for Medical Exemption

Directions to request a Medical Exemption:

• Your healthcare provider must complete the NJDOH Medical Exemption Request Form
• Upload the completed request form to the Student Health Portal in the "Document Upload" section as Medical Exemption (Immunizations ONLY)
• Fill out the Medical Exemption portion of the Immunization History Form located in the "My Forms" section of the Student Health Portal
