Before Your Visit

It is mandatory that all patient forms are completed prior to your appointment, whether it is in-person office visit or a remote telemedicine visit.  Please visit the Student Health Portal to begin.  Failure to complete your forms in a timely manor before your visit  will result in the visit being cancelled or rescheduled.  

All students must bring their Kean University ID and their health insurance card, if necessary for the appointment, with them to Student Health Services on the day of their appointment.

Required Forms for Students Under Age 18

Students under the age of 18 require parental consent for medical evaluation. If you are under the age of 18 at the time of your appointment, your are required to print the Consent for Treatment Form and have your parent or legal guardian complete it before your appointment. Upon completion upload form to the Student Health Portal. Minor students can not be seen for medical evaluation without consent.

Request A Copy of Your Medical Record

You have the right to access your medical records.

Our  health care providers may send a copy of your record to another provider only as needed for continuation of treatment or care with your permission.  An Authorization to Release or Exchange Confidential Information Form must be filled out completely in order to process this request.
